Typically, if someone has a Short Term Disability, Accident, Hospital Indemnity, or Critical Illness claim, they are told to fill out the information on the carrier's website or on documents that they then have to fax directly to the carrier.

Many times this process works out yet, we all know things usually don't go as smoothly as we would like. Because claims documents are made up of several parts and each part needs to be filled out by different people,  typically the employer, the employee, and the physician, it is nearly inevitable that something is missing. Claims are most often held up or denied due to lack of information, inconsistencies, or missing documentation.
